Tried it again, this is response I got; QuestionnaireAction is no longer required as process is already completed.

Did anyone got such message and later was able to access the questionnaire?

Probably You'v mistakingly submitted it....try Copying the link to a new Tab and see if it goes through. Try, Google Chrome.

@yondaimen, hw r u 2day? I think ur 2sensitive 2people's mistakes. While it's good u correct people, i think u can avoid becoming 2sensitive by glossing over them sometimes. My observation.
