I know u just saw a sample of a cover letter online and u want 2 follow every detail. Well, if u dont have info concerning the recruitment officer's name u can just address him/her as Sir/Madam. And the address part also is not important except where u actually have an address...

My friend, the contact of the person you address a cover letter to is critical in an application process. A well written cover letter goes a long way in separating you from the crowd and personalizing it sure makes the difference. We have to take that initiative, which proves us to be resourceful, to address the letter to an individual (name and tittle inclusive) and not just the HR dept.
