@johnpaul its not compulsory to collect d form in ur state of origin in d headoffice in LAGOS AT OJODU the form is been given accordin to ur state of origin, lagos, abia n adamawa were slated for collection today, married pple r not allowed, any form of sight problem or usin glasses is not allowed n some other conditions before ur #1000 will be considered. Cheers to all who will be applying me na average height

@Johnpaul, Maybaby is correct is not a must u ll trvel to ur state of origin to get the Form.

For those who bought the form do they request the letter of identification before issuing the form or when you are submitting the form. pls help!!!

2. medical fittness certificate.

u'll present d letter of identification,medical fitness certificate n other credentials b4 d form will b issued to u,that's d originals of ur credentials

Al dat na formality 4 those dat dnt know anybody,only height is looked at then credentials screened then u ar cleared to go 2UBA to make d 1k payment

@lanyat,even if u know GEJ that should be after submission of d form u purchased or does knowing people stop u from buying d form? ur knowing people shud take effect from abj n shortlisting of names
