Description:

The Wellness HealthFacilitator responsibilities include the following:
· Planning, designing, implementing andevaluating an on-going health and wellness program
· Linking resources and buildingcollaborative ties with clinical programs and wellness networks
· Overall management of healthincentive program
· Provides primary and preventativehealth education information
· Motivates CLIENT to adopt healthylifestyle behaviors
· Provides one-on-one and grouphealth counseling to help change unhealthy
behaviors
· Coordinates health fair, and otherhealth improvement activities, such as walking and weight loss programs
· Identifies community resources toassist with client referrals
· Serves as a health and wellnessresource, presenting lectures and discussion on health-related topics asrequested
· Coordinates blood pressure andcholesterol screenings, body fat analysis, and flu shot clinics
· Develops tracking tools to measureclient health improvements
· Keeps track of employees who becomeeligible for health incentive awards
· Works with healthcare provider tounderstand and communicate health benefits to employees
· Maintains confidential healthrecords of clients who participate in health improvement programs
· Using claims data develops andmonitors a strategic client wellness plan that produces measurable results
· Travel required between multiplelocations (throughout Nigerian and surrounding Countries) < 25%

OND,HND,BSC Required: preferably in Food Science, Biochem, Microbiology,Chemical Engineering, Health Education, Nutrition, Science related field
