In Nigeria here, you don't have to submit your form after you fill it completely................just print it out and make copies that you will carry to abj for the interview.All the best

The medical clearance cert must be from any teaching hospital or government recognized/own hospital.
As per the transcript, i don't know how you will go about it!!!
