1. Result Print-Out for 2011 JAMB UTME.
2. Result print-out for SSCE/NECO (Awaiting result accepted).
3. Two Passport Photographs.
4. Letter of sponsorship, stating the commitment to pay the prescribed fees.
5. Reference letter from a spiritual mentor, vouching for candidate’s character.
6. Evidence of purchase of Salem University Application form for N5, 200.
