I checked First Bank yesterday to enquire about the form, they have not recieved the card yet, i also sent a mail to ptdf to enquire when the form will be availiable, waiting for their response. Please let us post valid information here that will not mislead pple. IS THE FORM TRULLY OUT? FROM MY ENQUIRES NOTHING YET. PLEASE IF ANY BODY HAS VALID INFORMATION PLEASE LETS US KNOW.
