UST POST GRADUATE FORM ~HELP ME OUT

House, pls i just purchased the UST 2O10/2011 POST GRADUATE scratch card. Am having difficulties filling d form because i can't find the link in their site inorder to fill the form and download completed form for submission. Pls anybody with useful information should help me. Its urgent. Thanks
